Genetics was discussed repeatedly at the Boston Lymphatic Conference.

As research evolves, staying connected to scientific meetings and peer-reviewed literature is one of the best ways to stay informed about lipedema. This is true even for providers.

Lipedema often runs in families. Research consistently shows that lipedema clusters within families.

About 50% of individuals report a family history, suggesting a strong inherited component. Pedigree analyses have proposed patterns consistent with autosomal dominant inheritance with sex limitation or X-linked dominant inheritance, which may help explain why the condition almost exclusively affects women.

Lipedema also commonly appears during periods of hormonal transition, including puberty, pregnancy, and menopause. This has led researchers to investigate how estrogen may influence the expression of genetic susceptibility.

Lipedema is likely polygenic.

Rather than a single gene causing the condition, current evidence suggests that multiple genes contribute to risk.

Genome-wide association studies have identified several genetic regions associated with lipedema, including loci near VEGFA (involved in vascular biology) and GRB14-COBLL1. Other research has identified variants near LHFPL6, a gene previously linked to lipoma formation.

Hormone-related genes are also being studied.

Genes involved in steroid hormone metabolism, including AKR1C1 and AKR1C2, have been identified in some individuals with lipedema. Variations in these genes may influence how hormones are metabolized within adipose tissue.

These findings support the hypothesis that lipedema may be an estrogen-regulated genetic condition in which hormonal signaling interacts with inherited susceptibility.

What about genetic testing?
At this time, no single causative gene has been identified, and there is no validated genetic test available to diagnose lipedema.

Current evidence suggests lipedema is a complex condition influenced by multiple genes, hormones, and environmental factors, rather than a single gene. This does not mean it is not genetic.

Vitamin D & Lipedema: What the Research Shows

Vitamin D is often discussed in the lipedema community, but current evidence suggests the relationship is complex and mostly associative, not causal.

Vitamin D is a fat-soluble nutrient stored in adipose tissue. When fat mass increases, circulating vitamin D levels in the blood often decrease.

Because lipedema involves disproportionate subcutaneous fat accumulation, lower vitamin D levels are commonly observed. This may reflect how vitamin D is stored and distributed in larger fat depots, rather than vitamin D deficiency causing lipedema.

Population studies also show that lower vitamin D levels are associated with greater fat accumulation in the limbs, which mirrors lipedema distribution. However, correlation does NOT equal causation.

Another important finding:
Research shows vitamin D levels may drop after lipedema liposuction, sometimes into the deficient range, regardless of how much fat is removed. This suggests adipose tissue changes may influence vitamin D dynamics in the body.

Do vitamin D supplements treat lipedema?

Current research shows no evidence that vitamin D or other supplements improve lipedema symptoms or disease progression.

What the evidence does support:

• Screening for vitamin D deficiency
• Monitoring levels around lipedema surgery
• Supplementing when needed for general health

Right now, vitamin D should be viewed as supportive for overall health, not a treatment for lipedema.

Lower vitamin D levels are commonly observed in lipedema, but the research suggests this likely reflects adipose tissue biology rather than a causal role.

Lipedema tissue is biologically active, characterized by vascular changes, immune signaling, and inflammation.

One of the most common comorbid conditions reported in women with lipedema is mast cell activation. Many patients experience symptoms consistent with mast cell mediator release, including:
• flushing
• itching
• swelling
• headaches
• GI symptoms
• histamine sensitivity

This topic is also personal for me. I was diagnosed with Mast Cell Activation Syndrome (MCAS) in 2015, long before my lipedema diagnosis. Living with both conditions gives me a unique perspective as both a patient and a registered dietitian when considering these overlapping pathways.

Importantly, mast cell activation isn’t always constant.

It can flare and calm down depending on triggers such as inflammation, hormones, gut health, and immune signaling.

Here’s where oxygen becomes part of the conversation.

As adipose tissue expands, oxygen delivery depends on the microvascular system's ability to adapt. When tissue growth outpaces the blood supply, areas of localized hypoxia (low oxygen) can develop.
Research suggests lipedema tissue may become relatively hypoxic due to microvascular dysfunction and structural tissue changes.

Lipedema tissue has been shown to have:
• fragile capillaries
• increased vascular permeability
• extracellular matrix remodeling

These changes may impair the movement of oxygen and fluids through the tissue.

Why does this matter?

Because fat metabolism requires oxygen.

Processes like mitochondrial energy production and fatty acid oxidation depend on adequate oxygen availability.

Low oxygen → inflammatory signaling → mast cell activation → histamine release → increased vascular permeability → more swelling → further impaired oxygen delivery.

In immunology research, low oxygen levels activate HIF-1α, a pathway that stimulates inflammatory mediators and supports mast cell activation.

While HIF-1α's involvement in mast cell biology is well established, this hypoxia-driven pathway remains a biologically plausible mechanism in lipedema patients that researchers are investigating.

💦 Your lymphatic system doesn’t have a pump. Unlike your heart, it relies on muscle contraction to move lymph fluid. When you move, your muscles help push lymph through vessels, supporting fluid balance, immune regulation, and tissue health.
But it goes deeper.
When muscles contract, they release signaling proteins called myokines. These messengers help guide immune cells into lymphatic vessels, support vessel structure, and regulate immune activity in lymph nodes. Some even cross the blood-brain barrier, influencing inflammation, mood, and nervous system balance.
